Please print the waiver form below to complete and sign. You may bring this form with you to the Volunteer Orientation or on your first scheduled volunteer date.

HFH DeKalb Release of Liability and Waiver 

All volunteers must complete and sign this waiver. Waivers are good for one full year.

MINORS: Waiver forms for volunteers under age eighteen must also be signed by a parent/guardian. Minors who will not be accompanied on site by a parent/guardian must also have a notarized "Minor Consent to Treat" form on file in order to volunteer on site.

All minors must have a notarized copy of this form on file to volunteer when not in the company of a parent.
